Goal-based Task tracking and smarter document reading for Workers

Tasks are tracked by the original request, success criteria, verification results, and open items rather than as a checklist of steps. Scope is preserved when you request changes midway, which prevents unfounded completion. Earlier completion records are marked as lacking verification details.

Workers still read enabled Skills and required material once, but avoid re-reading what they have already read and unnecessary exploration, moving on to execution and verification sooner. Saved generation instructions and Custom Skill settings are supported.
