Stronger approval modes and audit logs for AI Workers

v1.1.84 makes AI Worker execution approvals easier to match to how your team works. Auto mode (no approval) and per-thread execution permissions let trusted work proceed without stopping each time. Every approval is recorded in a permanent audit log showing who approved what and when, so the history stays reviewable. Browser notifications now show the task name and let you approve on the spot.
